A CNIC applicant is required to produce the following documents at the time of application:

1. Birth Certificate or
2. Old NIC or
3. Matriculation Certificate or
4. CNICs of immediate/blood relatives
5. Citizenship certificate issued by MOI

Note :

1. No documents are demanded from illiterate applicant for age verification for first time.
2.  Residents of FATA/PATA will only be entertained at their native DAUs and their forms will be attested by concerned PA/APA.
